The C-DHQ II that queries intake for the past year is the standard version of the Canadian Diet History Questionnaire.
There is also a version that queries intake for the past month. Both versions are available in English and French. Please note
that the sample forms provided below SHOULD NOT be copied and completed by research subjects unless you intend to input the data
by hand. The codebooks for the paper forms are currently only available in English; however, the codes and column values may be used with
the French forms. The Diet Calc files are for use with the Diet Calc software which may be obtained from
the
EGRP Diet Calc download page.
A
demonstration version of the English past-year C-DHQ II can be
viewed. The French language and past-month versions are not available for demonstration but can be accessed for studies by contacting
IMS, Inc. (
